A certificate of insurance naming the "City of Phoenix as an Additional Insured" for $1 million dollars and the $100 permit fee ($5 million if shooting on an airport tarmac) is required. The following must be in the certificate holder's box.</p><p>City of Phoenix c/o Film Office<br> 200 W. Washington, 20th Floor<br> Phoenix, AZ 85003 </p>​​ <p>In the "description of operations section" above the certificate holder’s box it must read:</p><p> <em><strong>"The City of Phoenix is an Additional Insured with respect to liability arising out of the activities performed by or on behalf of the permittee."</strong></em>​<br></p><p>Your insurance company can email it to: <a class="strong" href="mailto:filmphx@phoenix.gov">filmphx@phoenix.gov</a></p><p>Please do NOT fax or mail a hard copy.</p><p>The permit must be obtained prior to the shoot.
