The City in no way warrants that the minimum limits contained herein are sufficient to protect the Permittee from liabilities that might arise out of the activities encompassed by the Permit and Permittee is free to purchase additional insurance as it determines necessary.<br><br></p><h2>Minimum Scope and Limits of Insurance</h2><h3> Commercial General Liability - Occurrence Form </h3><p>Policy shall include bodily injury, property damage and broad form contractual liability coverage.<br></p><ul><li>General Aggregate $2,000,000*<br><br></li><li>Products - Completed Operations Aggregate $1,000,000<br><br></li><li>Personal and Advertising Injury $1,000,000<br><br></li><li>Each Occurrence $1,000,000*<br><br></li><li>Fire Damage (Damage to Rented Premises) $ 50,000</li></ul><h3> Automobile Liability </h3><p>Bodily Injury and Property Damage for any owned, hired, and non-owned vehicles. Combined Single Limit (CSL) $1,000,000*</p><ul><li>The policy shall be endorsed to include the following additional insured language: "The City of Phoenix shall be named as an additional insured with respect to liability arising out of the activities performed by, or on behalf of the Permittee, including automobiles owned, leased, hired or borrowed by the Permittee."<br></li></ul><h3> Additional Insured required on each policy
<br></h3><ul><li>Both policies shall be endorsed to include the following additional insured language: "The City of Phoenix shall be named as an additional insured with respect to liability arising out of the activities performed by, or on behalf of the Permittee."<br><br></li><li>An excess liability policy or umbrella liability policy may be used to meet the minimum liability requirements set forth herein, provided that the coverage is written on a "following form" basis.<br><br></li></ul><p>
<strong>* These required limits shall be $5,000,000 for Permittee’s operations, if any, on the airside (“AOA”) of City of Phoenix airports.</strong></p><p>
<strong><br></strong></p><h2>Additional Insurance Requirements</h2><p>The policies shall include, or be endorsed to include, the following provisions:</p><ul><li>The City of Phoenix shall be an additional insured to the full limits of liability purchased by the Permittee even if those limits of liability are in excess of those required by the Permit.<br><br></li><li>The Permittee’s insurance coverage shall be primary insurance and noncontributory with respect to all other available sources.<br><br></li></ul><h2>Notice of Cancellation<br></h2><p>Each required insurance policy shall not be suspended, voided or canceled except after thirty (30) days prior written notice has been given to the City, except when cancellation is for non-payment of premium, then ten (10) days prior notice may be given. Written notices of Certificate of Insurance should be sent to:</p><p>City of Phoenix Film Office<br> 200 West Washington, 20th Floor<br> Phoenix AZ 85003<br><br></p><h2>Acceptability of Insurers</h2><p>Insurance is to be placed with insurers duly licensed or authorized to do business in the state of Arizona and with an “A.M. Best” rating of not less than B+ VI.<br></p>
<h2>Verification of Coverage<br></h2><p>Permittee shall furnish the City with certificates of insurance (ACORD form or equivalent approved by the City) as required at least 24 hours prior to the production. <strong>DO NOT SEND CERTIFICATES OF INSURANCE TO THE CITY’S RISK MANAGEMENT DIVISION.</strong></p><p>
<strong><br></strong></p><h2>Subcontractors</h2><p>Permittee’s certificate(s) shall include all subcontractors as additional insureds under its policies or Permittee shall furnish to the City separate certificates and endorsements for each subcontractor. All coverages for subcontractors shall be subject to the minimum requirements identified above.</p><p>

</p><h2>Approval</h2><p>Any modification or variation from these insurance requirements shall be made by the Law Department, whose decision shall be final.
